Chaudhry Pulmonary Associates works with primary care physicians, specialists, discharge planners, and clinical offices to support adult patients with pulmonary and sleep-related concerns.
Helpful Information to Include
Patient name, date of birth, contact information, insurance information, and reason for referral.
Recent office notes, medication list, imaging reports, pulmonary testing, sleep study records, or hospital discharge paperwork when available.
Briefly note the primary concern, such as COPD, asthma, chronic cough, shortness of breath, sleep apnea, oxygen needs, abnormal imaging, or post-hospital pulmonary follow-up.
Include the referring office name, provider name, phone number, fax number, and preferred method for follow-up communication.
Common Reasons for Referral
Pulmonary Consultation
For adults with respiratory symptoms, abnormal imaging, complex pulmonary history, or concerns requiring specialist evaluation.
COPD & Asthma Care
For patients with COPD, asthma, wheezing, recurrent flare-ups, medication concerns, or ongoing breathing difficulty.
Chronic Cough & Shortness of Breath
For persistent cough, unexplained shortness of breath, abnormal breathing symptoms, or symptoms not improving with initial treatment.
Sleep Apnea Evaluation & CPAP Support
For suspected sleep apnea, snoring, daytime fatigue, CPAP intolerance, mask issues, or ongoing sleep-related breathing concerns.
Oxygen Therapy Management
For patients using oxygen therapy or needing evaluation, monitoring, or follow-up related to oxygen requirements.
Post-Hospital Pulmonary Follow-Up
For patients recently hospitalized with pneumonia, COPD flare-ups, respiratory illness, oxygen needs, or other pulmonary concerns.

Send the referral request and relevant clinical information by fax.
Office Review
Our office reviews the referral information and contacts the patient or referring office as needed.
Scheduling
The patient is scheduled for the appropriate pulmonary or sleep medicine visit.
Follow-Up
Follow-up communication is handled based on the patient’s care plan and clinical needs.
